The cost of the i693 medical examination is $550. This price covers the examination fee and lab work. Additional cost for vaccines.

Insurance is not accepted for the i693 medical examination. However, a receipt can be provided to you upon request for you to submit to your insurance provider.

The Tests/Vaccines that are required include: • Polio (vaccine) • MMR (vaccine or blood titer) • Hepatitis A and B (vaccine or blood titer) • Varicella (vaccine or blood titer) • RPR for Syphilis (anyone between the ages of 18-44) • Urine Gonorrhea NAAT (anyone between the ages of 18-24) • Quantiferon Gold TB Test (anyone over the age of 2 years) • Flu vaccine (during flu season, the months of September - April)

All lab tests are covered in the cost of the examination. Vaccines are not covered but low cost options will be provided.
